Medical Records Request

Please download the Authorization for Release of Patient Information, below, complete all required fields, sign it, and email the form to results@privydiagnosticslabs.com. Records older than seven years old are discarded according to state and federal guidelines. A licensed provider shall maintain adequate medical records of a patient for a minimum of seven years from the anniversary date of the date of last treatment by the provider. CMS requires Medicare managed care program providers to retain records for 10 years.
